You may try looking at PMBOK. They have samples and I've even seen some on the net.

I would check out the ReadySET Templates. Risk Management is part of the Project Plan, a change log is part of the Release Notes, and issue tracking is part of the QA Plan and Test Suite documents.

Steve McConnell has a huge collection of resources on his website covering every area of the SDLC, look under the CxOne Basic area for starters, its a free sign up but you get access to a huge wealth of information.

These documents all have guiding statements and examples and are very useful when starting or trialling new concepts.

There are also Microsoft Solutions Framework sample project lifecycle deliverables available for download.

Generally, you can start with some external templates, but then you'll have to to customize and tune them based on specific needs of your organization or even individual project. It makes no sense to fill parts of documents, which are never used later in the project.
