SilverLight Out of Browser Problem

Question

I am new to SilverLight and trying to use the "Out of Browser" option. I have selected "Enable running application out of the browser" and set settings, windows title, etc. The app compiles and runs without errors but I do not get the option to install when right clicking in the browser (just the standard SilverLight Settings option).

I am using Visual Web Developer 2008 Express Edition and have double checked that the OutOfBrowserSettins.xml is present and that the project file references this new file. What could I be doing wrong? If I go to other SilverLight apps with this option I get the "Install" option when right clicking so I know the SilverLight runtime is working correct. Any ideas?
