I've looked at the Umbraco Projects page but can't see anything relevant there. I would like to be able to create surveys (add questions and answers) and store survey results somewhere. I realise this isn't rocket science and I could just build something myself, but if there's something already out there that would allow me to do this, I'd rather not reinvent the wheel.

They do surveys as well as polls - for free. All you do is configure it, and then get an embeddable code snippet that you put on your page. You can customize the appearance through CSS.

polldaddy looks good, as do many of the other online survey apps. I don't want someone else to host my survey though.

Although not a specific survey engine you could use Umbraco Contour. This is a form builder which would allow you to create a survey form and save the data in umbraco, in a db etc.
