What works best for you and how do you collate the feedback?

one good way is prototyping. Build a prototype of what you want to build and get a couple users in to test the product prototype. Observe what the notice and make sure you get them to talk while they interact.

Once you understand what you need in your product you can start to build the actual product. Going into a private alpha/beta is a great way to get feedback on your product. Make sure to either create a tool or use a tool like (Get Satisfaction) to get user feedback.

I'm using feedback form provided by UserThought.com for my website.

Theirs feedback form is customized and integrates with just one line of HTML code.

The best and unique feature (I realize) is content highlighting. It allows my users to highlight some fragment of my webpage together with feedback message. That points me directly to what user meant in the comment.

Also rich feedback analytics helps to understand users satisfation & loyalty trends.

I'm using it with my website for a long time and I'm very happy with theirs service.
