Does anyone know a good tutorial for this subject. I want to create a module (extension) with more than one page. The extension must contain one overview page and a form to create a item.

I'd second Alan Storm's tutorials. We found them invaluable when tooling up on Magento recently at our work. They don't however go as far as how the admin forms work, which seems to be what you're asking about. I'd also be interested in something that goes through this area of Magento development.

That site seems to be about general PHP techniques. While interesting, it doesn't help with Magento modules, which are complex enough to require specific help. Hence the poster's question.
