  • Optional. You don't need a bank account if your plan is to go with free apps. Though with iAd in the future, you may want it just for the ad revenue.
  • Required. The public will never see an email address unless you want to publish it. Apple will need a support email address to use for issues with approval, on-going issues, etc.
  • Required. You will need a support website. This doesn't have to be very complicated. Apple just wants a URL. How put together this website is may impact how well your app does but from a requirement standpoint, any website will do.
  • Optional. You don't need a company name. Your apps can be published under your own individual name.
  • Optional. Support phone number isn't required. My experience is that all the interactions with Apple are done over email. This also creates a nice paper trail where phone calls don't leave anything.

You don't need to form a corporation. A simple "doing business as" (commonly referred to as a DBA) from your local government should suffice.
